How to enable Cookies

Please follow the instructions below for the browser version you are using.

Microsoft Internet Explorer 6.x and 7.x

  • Select “Internet Options” from the Tools menu.
  • Click on the “Privacy” tab.
  • Move the cookie level to “Medium”
  • Click “OK”.

Mozilla Firefox 3.x

  • Select “Tools” –> “Options” from the top menu,
  • Click on the “Privacy” tab,
  • Tick/check where it says “Accept cookies from sites” and “Accept third-party cookies”
  • Click “OK”.

Safari 4.x

  • Select “Preferences” –> “Security”
  • Check/tick “Only sites I visit” under the “Accept cookies” section
  • Click the ‘x’ to close the window.

Microsoft Internet Explorer 5.x

  • Select “Internet Options” from the Tools menu.
  • Click on the “Security” tab.
  • Click “Custom Level” button.
  • Scroll down to the “Cookies” section.
  • To enable:
  • Set “Allow cookies that are stored on your computer” to “Enable”.
  • Set “Allow per-session cookies” to “Enable”.
  • Click “OK”.

I can't download/view any of your forms

We have developed the documents for this website in Portable Document Format (PDF). To be able to view PDF files in your browser, you must have the Adobe Acrobat Reader installed on your machine.

You will know if you have this installed (or not) when you double click on one of the documents. If the document does not appear, you must click on the image on the left and you will be redirected to the Adobe website to download the Acrobat reader. To start the installation, click on the ‘Download’ button. Please note that different operating systems (Vista, Windows XP, windows 98, etc) may have different download packs so please read the adobe instructions carefully to get the correct version for your system. It will take about five to ten minutes, depending on your modem/broadband speed. It only has to be installed once.

How to enable Javascript

If JavaScript is turned off in your browser, please follow the instructions below for your browser type to turn JavaScript on. (Once done you will need to refresh the form page by pressing ‘F5’ or the refresh button on your browser)

Internet Explorer 6.x

  1. Access the Tools menu and select Internet Options
  2. Click the Security tab
  3. In the Security dialog, click on the globe for “Internet” zone
  4. Click on the button labeled “Custom Level”
  5. In the dialog that appears, scroll down to the heading called “Scripting”
  6. Click the “Enable” button under “Active Scripting” to turn JavaScript on

Mozilla Firefox (1.0)

  1. Select ‘Tools’ from the top menu
  2. Choose ‘Options’
  3. Choose ‘Web Features’ from the left navigation
  4. Select the checkbox next to ‘Enable JavaScript’ and click OK

Mozilla Firefox (1.5)

  1. Select ‘Tools’ from the top menu
  2. Choose ‘Options’
  3. Choose ‘Content’ from the top navigation
  4. Select the checkbox next to ‘Enable JavaScript’ and click OK

Apple Safari (1.0)

  1. Select ‘Safari’ from the top menu
  2. Choose ‘Preferences’
  3. Choose ‘Security’
  4. Select the checkbox next to ‘Enable JavaScript’

Please keep in mind that upgrading your browser or installing new security software or security patches may affect your JavaScript settings. If you are still experiencing problems viewing the ads after ensuring that JavaScript is enabled in your browser, please check if you have any personal firewall or security software installed.
